Brooklyn Interagency Case Conference

Please join the Brooklyn Consortium as we bring together professionals across sectors (homeless outreach, supportive housing/social services with hospital ED clinicians, inpatient social workers, discharge planners, etc.) to discuss challenging cases and collaboratively troubleshoot solutions for patients and clients with complex health and housing needs. The case conference will be facilitated by Casey Burke, Street-to-Home Director at Breaking Ground.
