The Bronx Health & Housing Consortium (the Consortium) organized in 2011 with the purpose of establishing a collaborative network of providers in the Bronx with the shared goal of strengthening and integrating the healthcare and housing systems at the policy, operational, and individual client/patient levels. In 2017, we incorporated as The Health & Housing Consortium, Inc. and in 2018 expanded our model from the Bronx to Brooklyn.

The Consortium is comprised of representatives from more than 60 hospitals, health homes, community-based organizations, managed care plans, government agencies and more. The range of services provided by these groups includes housing, community-based health care, social services, behavioral health, harm reduction, case management, and advocacy, to name but a few.
